How to refer to one of our therapists:

  1. Please complete the referral form below, but only if you have received permission from your client/patient.

  2. You will receive a confirmation email that we have received your referral.

  3. If you would like to fax any additional information, please send to 616-828-0293

  4. Your referral will receive a returned email within 1 business day (from our Intake Coordinator via hello@theflourishingco.org). You can also call or text our office: 616-209-8539 if you need support or have questions.
